IN WARRANTY (within 1 year of ship date)
- Matthews will replace the controller and all of the installed licenses free of charge as long as there is no physical damage to the controller.
- If immediate replacement is needed, please provide a new purchase order for the controller and all required licenses. Email the P.O. to [email protected].
OUT OF WARRANTY
- If there are no licenses on your controller, you will need to purchase a new controller. Email your PO to [email protected].
- If licenses have been installed on the controller, complete the information on this form to obtain an RMA No.

A valid Purchase Order is required with the Return Authorization/Repair Request for all items.
To readily identify your package, display the Purchase Order Number and any other information requested on the shipping label, and include it on all corresponding documentation. Ship to 3159 Unionville Road Suite 500, Cranberry Township, PA 16066.
Repairs that do not receive an approval or Credit Card information after one week of quote will be invoiced the $250 Bench Fee and returned to you with freight prepaid and added to the invoice.
Pre-authorized repairs will be automatically invoiced upon shipment of completed repair.
The customer is responsible for shipping and insuring equipment to and from the Matthews Service Repair Center.
Please review Matthews instruction manuals for proper preparation of packaging your equipment for return to Matthews. The customer is responsible for any damage incurred due to improper packaging.
Warranty will be determined after inspection of equipment by a trained Matthews technician per our policy.
Items that have been subjected to severe misuse/abuse, modification, or those that exhibit catastrophic failure characteristics, cannot be accepted under the terms of this program. A $250 bench fee will apply.
Matthews does not provide equipment to the customer while the customer equipment is being repaired unless explicitly stated by a pre-arranged agreement.
Matthews provides a 90-day warranty for repaired equipment.
Please do not ship any inks on a RMA. Please contact Matthews Customer Service at [email protected] to obtain a Return Authorization for ink.

Matthews warrants that the Equipment will be free from defects in material and workmanship for a period of THREE HUNDRED SIXTY-FIVE (365) days from the date of shipment from Matthews to customer. This warranty is applicable only if the product has had normal utilization within its specifications, has been utilized with Matthews approved parts and supplies, has not been modified or altered in a manner not approved by Matthews, and has been maintained in accordance with recommended maintenance procedures. Matthews liability shall be limited to the repair or replacement of the item in warranty after return prepaid and packaged to Matthews factory or authorized repair facility. The Equipment will not be returned to Matthews without its written consent. Return of repaired unit to Buyer or the end-user shall be at Matthews expense.
